I generally use iTunes (error correction on) and rip into 'ALAC'. Why? Good sounding results. Glitchy tracks/cds can be attacked with Max Comparison ripper (seems to work best for me). edit: I now use Max for all non-pristine discs (and that means any visible surface anomalies whatsoever) I like iTu...

Least and Most compressible tracks in Apple Lossless
My least compressible track so far has been Sadame, by Japanese band High Rise at 1392kbps in MAX's Apple Lossless format. The greatly compressible have been Stephan von Huene's "Extended Schwitters" at 202 kbps, and "Reprise of the infernal machine" by Sh-Man-Tra at 176kb/s , as reported by iTunes....

Like many max users I am in the process of ripping my CD collection to a lossless format. Things have been going quite smoothly but there have been a few weird things happening. I have a CD that does this as well. Not quite 'all noise', but definitely very very wrong. When I try to use Paranoia-ripping on The Necks' "Chemist" to produce AIFF and/or m4a I just get junk.
